HFH support those who need short-term provision such as people returning from hospital or families needing respite care to providing longer term care, for example, to older people, people with disabilities and those with chronic or even terminal illness. HFH provides bespoke care packages for people requiring palliative care. We recognise that people who are dying require a service that is responsive to their needs and we are able to provide this service at short notice and when needed.
We also know that this is a time when family members require immense support and our competent staff help to achieve this by undertaking domestic tasks or providing respite care to enable family members to take some very important 'time out'.
HFH palliative care team is nurse led. Our team has received expert training from St. Christopher's Hospice and Nescot College.

At HFH we recognise that people may need that extra bit of care and support in order to remain independent in their own home and part of the wider community. Extra care housing provides the means for vulnerable people to achieve this.
HFH provides 24 hour care and support in Chestnut House extra care housing scheme in partnership with Wandsworth Council and Richmond Upon Thames Churches Housing Trust. Our clients feel safe and secure within this environment and the wide range of activities available has enabled them to flourish and remain part of the wider community.
